If he genuinely wants her dead, she's going to die unless she escapes first. If he doesn't want to kill her unless necessary, he will feel like he got a free car as she runs out. If he's going to start shooting when she escapes, he would have shot her regardless, so your best chance would have been to plan an exit, even if you get shot in trying to do so.

Really just drive to a crowded space or police station, or refuse to drive and throw the keys away. Really hard to know what to do when you have a gun in your face.
