Dealing with the top 0.15% of results, you've got so few results that it becomes pretty much impossible to categorically differentiate between them.

If you get 100% of questions correct on an IQ test, you're pretty much going to be told you are 142+ or 145+ because the sample size is far too small to be able to differentiate between anything greater.

The fundamental problem is that we know according to statistics there are roughly 16 people living today who have a IQ of 190+.
To actually find them out, would require a test that only those people score 100% on and over 8 billion people on the planet has to take. Of course no such test exists.
