Okay, I almost explained this earlier. I used to work at mcshits, and the reason they don't expand mccafe is because mccafe items require the use of entire new machines. This takes time to prepare, which adds on to the time it takes to prepare burgers in the drive through. This wouldn't be an issue if you didn't have burgers and an entire staff designed to handle the grill item construction, but you do, so now when someone orders something from mccafe, it's actually a strain on the workflow of the people to make it. Even just a smoothie or an icecream (the real reason the machine is "down" a lot is because no one wants to take the time to clean it.)

It's much easier just to open a new place.

It would have worked if it were allowed to be tiny, like four tables max or drive-thru only, don't prepare food on-site, squeeze into gas station lots or store-in-store in large grocery stores, and sell McDonald's coffee at McDonald's prices. They could have just spammed tiny locations that are cheap to set-up and operate, and the pricing and the scale of economy would have killed all remaining mom-and-pop coffee shops that couldn't move up the value chain and compete on quality.
